Attention former subscribers and individual ticketholders of New York City Opera, Inc. (“NYCO”):
On or about January 27, 2016, a “Notice of Entry of Disclosure and Confirmation Order and Effective Date of the Plan” was mailed to all known former subscribers and ticketholders advising that NYCO’s Chapter 11 Plan had been confirmed by the Bankruptcy Court and requesting all such subscribers and ticketholders to submit W‐9 Forms within 60 days (i.e. March 28, 2016). The Forms must be mailed to the following address:

If you have not already done so, please send in your W‐9 Form. We are currently reconciling returned W‐9 Forms with the books and records maintained by NYCO, and we anticipate that checks will be released to creditors beginning in April 2016.
